Nursing Care Plan Template with Examples (Student Version)
A student nursing care plan has five linked parts: assessment data, a nursing diagnosis, a measurable goal, interventions and evaluation. Each should connect to the one before.
The short answer
The diagnosis comes from the data, the goal answers the diagnosis, interventions serve the goal, and evaluation checks the goal. The exact template and depth vary by school, so follow your program's rubric.
A filled-in example (fictional patient)
Assessment: Patient reports pain 7/10 in right hip after surgery, guarding movement, reluctant to ambulate.
Nursing diagnosis: Acute pain related to surgical incision, as evidenced by self-reported pain score and guarding.
Goal: Patient will report pain of 3/10 or less within 30 minutes of intervention, by end of shift.
Interventions: Give prescribed analgesia; reposition for comfort; teach splinting for movement.
Evaluation: Reassess pain score 30 to 60 minutes after intervention; document response and adjust the plan.
Common mistakes students make
- Writing a goal that is not measurable ("patient will feel better").
- Choosing a diagnosis the assessment data do not support.
- Listing interventions with no clear link to the goal.
- Skipping the evaluation step or leaving it vague.

Where Fiabita fits
Fiabita's Study Lab turns your own course notes into quizzes and flashcards, and its "Clean up my note" tool can help you reword a practice note in clearer professional language. Everything it produces is a draft for you to check against your course material, your instructor and your school's rubric. Do not enter anything that could identify a real patient, and do not treat the output as a substitute for feedback from your instructor or preceptor.
FAQ
Q: Do all Canadian nursing programs use the same care plan format?
The five-part structure is widely taught, but the exact template and expected depth vary by school. Follow your program's rubric.
Q: How specific does the goal need to be?
Specific enough that another nurse could tell, without asking you, whether it was met. Include a timeframe and a measurable outcome.
Q: Can Fiabita write my care plan for me?
No. Fiabita can help structure your own assessment data, but the clinical reasoning and final submission are yours to verify with your instructor.
